| Case Management |
The Case Management program provides frail seniors with the opportunity to make their own choices regarding long-term care. Case Management offers frail seniors age 60 and over an alternative to nursing facilities or other forms of institutionalized care. The Case Management program helps coordinate community services which will allow older people to remain safely in their home. Each person is monitored by an individual Case Manager who provides assistance in making the appropriate choices that best fit their needs. Case Management Process:
There may be a fee for services received. Some services may be covered by Medicare and private insurance. The Medicaid Elderly Waiver program available through the Department of Human Services may help those ages 65 or older who meet medical, income and resource guidelines.
