Staying at Home
One of the key concerns for the future identified by older Iowans and their caregivers is the hope that they will be
able to stay in their own homes - with the help and support to make that possible.
They know that the time may come when they or their loved one will need help with daily living activities like housekeeping, chores, medication
management or personal care.
Many organizations can help with some or all of these needs. But finding a workable and affordable combination
can be a difficult, even overwhelming task.
Using a Care Management System can help Iowans identify, secure and coordinate the services
they need to stay in their own homes.
How Does It Work?
An experienced counselor will visit your home and a comprehensive
assessment is completed.
A detailed plan of care is developed and reviewed with the senior and caregivers.
Once the senior and caregivers involved approve the care plan, the care manager
will make the necessary calls to ensure that the plan of care serves the identified needs.
The care manager continues to work with the senior and caregivers as long as necessary.
What Does It Cost?
There is no cost for care management assessment and coordination. Agencies providing care may charge for their
professional services.
Some services may be covered by Medicare and private insurance. Many agencies use a
sliding fee schedule for their services.
The Elderly Waiver program, available through the Department of Human
Services, may help those age 65 or older who meet medical, income and resource guidelines.
At no time will you be obligated to pay for something you did not authorize.
Who is Eligible?
Hawkeye Valley Area Agency on Aging, offers care management services to seniors age 60 and older
in Black Hawk, Bremer, Buchanan, Butler, Chickasaw, Grundy, Hardin, Marshall, Poweshiek and Tama
counties.
The Care Management System is a Case Management Project for Frail Elderly and is funded in part
by the State of Iowa and the Older Americans Act.
